Manitouwadge Family Health Team “Team Talks!”

MANITOUWADGE, ON – Many new and exciting events are taking place at your Manitouwadge Family Health Team (MFHT), where the team strives to bring you quality healthcare opportunities through resources and support to help live a quality life.  The MFHT programming is really starting to take shape as all our service providers are excited to establish partnerships and work with the community to improve their health.  A prime focus of the team is staying true and committed to their Vision, “Meeting the changing health care needs of our community with excellence”.

The MFHT is funded by the Ontario Ministry of Health and Longterm Care.  The entire planning process took many months to implement and the MFHT finally opened its doors to the community on May 24, 2011.  These teams have been put in place by the government for a variety of reasons including, “providing health information so that patients can make informed decisions on how to manage their health care needs, support health promotion and disease prevention activities as well as the management of chronic diseases, such as diabetes and asthma, to improve the coordination of health care by better linking our patients to other parts of the health care system, such as:

  • hospitals,
  • long-term care facilities,
  • public health,
  • mental health and
  • community programs and services,

to use information technology more effectively, resulting in health care providers across the system sharing appropriate, timely and secure access to patient test results and medical histories.” (Ministry of Health website)

All of our service providers will use their knowledge and guidance to provide you, the patient, with the best care.  We currently have various programs running including a Hypertension Management Program, a Diabetes Management Program and Aqua Aerobics at the pool.  We have provided summer exercise programs for older adults at the Golden Age Center and at Lakeview Terrace which we hope to continue in the fall.  MFHT providers are currently working on an EPIC Series program titled Empowering Healthy Lifestyle Choices which will include various sessions such as Body Balance, Smoking Cessation, Heart Health, Stress and Coping and Diabetes Management.  The MFHT works collaboratively with the Manitouwadge Medical Clinic and physicians to help provide direction in the design and implementation of our programs.  Best practice guidelines and adhering to the strictest confidentiality standards are of the utmost importance to the MFHT.  We will be using the same electronic medical record system as the medical clinic and by giving your consent, will be able to share your medical records with the clinic allowing for the best continuity of your health care.

A referral is not mandatory in order to become a patient of the MFHT.  If you simply have questions regarding your health, you are able to make your own appointment with any of our service providers.  We are not a substitute however, for acute conditions of sudden onset of pain or illness or conditions that have not been diagnosed by a physician.  For any of these situations like a sore throat or cold, abdominal pain or for prescriptions, please see your family physician.
